The stat about glue-down LVP failure rates in commercial kitchens blew my mind

I was reading a technical bulletin from RFCI and found out that over 60% of glue-down LVP failures happen because the subfloor moisture wasn't checked properly before install, and that really hit home after I did a job at a diner in Phoenix last spring where the tiles started curling within 3 months, has anyone else run into moisture issues with restaurants?

Wait isn't the RFCI stat more like 70-80 percent of all glue-down failures are moisture related? I remember reading that in their install guide too but I think it includes more than just subfloor moisture, like humidity during install and even the concrete not being cured long enough. Ive seen guys skip the calcium chloride test and just throw down flooring on new concrete that was only 2 weeks old, then wonder why it bubbles up. Moisture tests are cheap compared to ripping out a whole kitchen floor three months later.
