Health insurance headaches from country hopping are real

I got a bill from a clinic in Portugal because my US plan called it elective care since I was abroad. One side says dedicated nomad insurance is worth the cost, while others stick with travel policies and hope for the best. Filing claims feels like a part-time job with all the different rules. How do you guys handle medical coverage without going broke or crazy?

Portugal actually requires proof of specific travel medical insurance for some long stay visas (I had to look this up for a client once). So the "hope for the best" plan can literally block your entry if you're staying more than a quick trip. That emergency fund won't help at the border if your paperwork is wrong. It's not always about the math, sometimes it's about the legal fine print they don't tell you about.

That bit about "dedicated nomad insurance is worth the cost" seems like a trap. Those plans are crazy expensive and still have tons of fine print. I've used a basic travel policy for years and just pay out of pocket for small stuff. Saved thousands compared to my friend on a fancy global plan who still fought over claims. Sometimes hoping for the best and having a big emergency fund is just simpler math.

Why do we assume expensive insurance is BETTER, when your experience shows the FLAWS? I used to swear by it, but seeing friends fight claims while paying HIGH premiums changed my mind. Now a SOLID emergency fund with basic coverage seems like smarter math.
